Join Oakland Community Health Network (OCHN) as an Access Screener, playing a vital role in connecting individuals with mental health, intellectual and developmental disabilities, and substance use services. As a key member of our team, you will provide warm and welcoming assistance to the public, ensuring they receive the support they need.
Key Responsibilities:- Provide exceptional customer service, empathy, and support to individuals seeking mental health, IDD, and substance abuse services.
- Conduct thorough eligibility screenings, determining the appropriate level of care for individuals.
- Collaborate with internal and external providers to ensure seamless coordination of services.
- Link individuals with community resources, providing education on available services and benefits.
- Adhere to established Quality Improvement Indicators, ensuring high-quality service delivery.
- Master's degree in a mental health field, with a valid clinical license (LLP, LP, LMSW, LPC, LMFT, LLMSW, LLPC).
- 1 year of experience in Behavioral Health or Substance Use disorder services.
- Knowledge of OCHN provider network, community resources, and admission/discharge criteria.
- Strong interpersonal and communication skills, with the ability to articulate complex service systems.
- Excellent customer service and communication skills.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Strong organizational skills, with attention to detail and accuracy.
- Knowledge of trauma-informed principles and recipient rights policies.
