Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a PRN Home Health RN. As a member of our team, you will provide compassionate, patient-centered care to patients in their homes, working collaboratively with our interdisciplinary care teams to develop and implement personalized care plans.
Responsibilities- Coordinates and oversees all aspects of patients' home care, ensuring timely and accurate documentation of patient progress.
- Collaborates with patient care teams to develop and implement care plans, taking into account the unique needs and goals of each patient.
- Educates families on patient condition, needs, and care, providing support and guidance to ensure seamless care transitions.
- Performs head-to-toe physical assessments, checks vital signs, and manages medications, administering injections, IV therapy, or tube feedings as needed.
- Administers and maintains catheters, ensuring patient comfort and safety.
- Supervises Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), providing guidance and support to ensure high-quality patient care.
- Regularly communicates patient progress to the clinical manager, physician, and care team, ensuring seamless care coordination.
- One year of clinical experience as a Registered Nurse (RN), with a minimum of six months of home health experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification and valid driver's license, with reliable transportation and liability insurance.
- Able to assess patient status and identify requirements relative to age-specific needs.
- Familiarity with general use and functions of the computer, including user names and password concepts, internet, email, navigation of computer desktop, and online HR services.
