About the Role

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our Home Health & Hospice team at Aegis Healthcare. As a key member of our team, you will play a vital role in delivering exceptional care to patients in their homes.

Key Responsibilities
  • Verify and execute the plan of care as prescribed by physicians and under the guidance of a Registered Nurse.
  • Provide essential nursing treatments, promptly address patient pain, and assist with activities of daily living.
  • Educate patients and caregivers on self-care techniques.
  • Administer medications, ensure the availability of necessary supplies, and maintain accurate patient records.
  • Liaise with supervising RNs or physicians on patient assessments and any changes in conditions.
Requirements
  • Current Licensed Practical Nursing License in the state of practice.
  • 1+ years of Home Health & Hospice experience preferred.
  • Ability to work independently, be self-directed, and flexible.
  • Ability to use a full range of motion and lift 35lbs.
  • Effective verbal & written communication & time management skills.
  • Valid Driver's License (clean record) & reliable transportation.
  • Proof of current auto insurance & registration.
  • Valid AZ Fingerprint Clearance Card (we can help).
  • Current CPR Certification (we can help) & Negative TB Test (we can test).
