Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Southern Home Care Services, Inc. as a Patient Care Specialist. In this role, you will be responsible for providing high-quality patient care to individuals in their homes, ensuring their dignity and independence are maintained at all times.

Key Responsibilities
  • Provide 1:1 care to clients, following their individualized care plans and maintaining their dignity and independence
  • Monitor clients' vital signs and report any abnormal findings to the case manager and physician
  • Administer medications, feedings, oxygen, and ostomy care as needed, and document the results
  • Complete routines established by other care professionals, such as Physical Therapists, Occupational Therapists, and Speech Therapists
  • Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections
  • Accurately document nursing actions and communicate with family, case manager, and physicians
  • Order supplies and medications as needed to ensure adequate amounts are on hand
  • Ensure all patient needs are met in accordance with physician instructions
  • Monitor incidents involving individuals and complete incident reports in a timely and accurate manner
Requirements
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse
  • Knowledge of individual service plans, including understanding of client rights and health needs
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice
  • CPR Certified
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion
  • In-depth understanding of patient care policies and regulations
  • Familiarity with symptoms and signs of improvement for several medical conditions
