Physical Therapist PRN

The Physical Therapist (PT) plays a vital role in providing high-quality, patient-centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team.
Key Responsibilities- Conduct thorough patient assessments and participate in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience.
- Provide purposeful and skilled therapeutic care at patients home, using procedures and techniques to produce a change in condition/function consistent with the physicians plan of care.
- Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Prioritize caseload, determine daily staffing needs, and coordinate assignments/handoffs.
- Graduate of an American Physical Therapy Association accredited school with a BS, MS, or Doctorate degree in Physical Therapy.
- Active Physical Therapy license in the State of Colorado.
- Current certification in BLS for HealthCare Providers (in accordance with American Heart Association guidelines).
- Successful completion of student clinical rotation or work experience in at least one of the following settings: acute care hospital, long-term acute care facility, skilled nursing facility, and/or acute rehabilitation setting.
