Job Summary

We are seeking a skilled LPN Case Manager to join our team at BRC, a leading provider of homeless services in New York City. The successful candidate will be responsible for providing case management services to individuals experiencing homelessness, including those with mental illness, substance abuse, and serious medical problems.

Key Responsibilities
  • Coordinate health care and medical-related case management for clients
  • Provide ongoing health monitoring to manage and support clients' health needs
  • Make appropriate referrals and follow-up for medical, substance use, and/or psychiatric treatment
  • Observe, monitor, and document client self-management of medication
  • Process physicians' orders and obtain prescriptions
  • Collaborate with medical and psychiatric providers and hospitals
  • Develop and facilitate health-related educational groups and one-on-one client education
  • Participate in interdisciplinary team meetings and case conferences
  • Respond to medical emergencies and provide emergency first aid/CPR, and Narcan opioid overdose reversal when needed
Requirements
  • NYS LPN license required
  • Bachelor's Degree preferred
  • Experience working with individuals experiencing homelessness preferred
  • Knowledge of mental illness and substance use disorder helpful
  • Proficiency in MS Office, including Word and Excel
  • Bilingual skills a plus
  • CPR training certification or willingness to complete certification training in CPR and opioid overdose prevention
