**Job Summary:**

The ClearChoice nursing team is seeking compassionate and skilled nurses to provide exceptional patient care and support at our nursing center. As a Nurse Care Specialist, you will work regular 8 hour shifts or as a Baylor nurse, 12 hour shifts from Friday through Sunday and will receive full-time benefits while only working 36 hours per week.

**Key Responsibilities:**

  • Provide personalized nursing care to patients, prioritizing their comfort and well-being.
  • Collaborate with the healthcare team to develop and implement patient care plans, ensuring seamless care coordination.
  • Administer medications and treatments as prescribed by physicians, maintaining accurate records and reporting any changes to the healthcare team.
  • Monitor and record patient vital signs, identifying any changes or concerns and communicating them to the healthcare team.
  • Educate patients and their families on disease management and treatment plans, promoting informed decision-making and self-care.
  • Assist with medical procedures and examinations as needed, maintaining a safe and respectful environment.
  • Document patient care accurately and in a timely manner, adhering to all hospital and nursing policies and procedures.
  • Maintain clean and organized patient rooms and equipment, ensuring a safe and healthy environment.

**Requirements:**

  • Active nursing license in the state of Florida.
  • Minimum of 1 year nursing experience in a skilled nursing facility (SNF) setting, preferred.
  • BLS and ACLS certification required.
  • Strong communication and organizational skills, with the ability to work effectively in a fast-paced and dynamic healthcare environment.
  • Compassionate and patient-focused attitude, with a commitment to delivering exceptional patient care.
  • Ability to work all shifts, including weekends and holidays, and pass a background check and drug screening.
