About the Role

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Doctors Hospital of Manteca. As a key member of our healthcare team, you will play a vital role in ensuring that our patients receive the highest level of care and support.

Key Responsibilities
  • Coordinate patient care across the continuum, ensuring seamless transitions and optimal health outcomes.
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure effective communication and coordination of care.
  • Identify and address potential barriers to care, including social determinants and environmental factors.
  • Provide education and support to patients and families, empowering them to take an active role in their care.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
  • Document all patient interactions and care activities in a timely and accurate manner.
Requirements
  • Graduate of an accredited school of nursing with a current RN license.
  • Minimum 5 years of acute hospital patient care experience, with a focus on case management.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work in a fast-paced environment, with a high level of adaptability and flexibility.
  • Knowledge of case management principles, practices, and standards.
