About the Position
TravelNurseSource is working with TRS Healthcare to find a qualified Case Manager RN in a dynamic healthcare environment. We are seeking a Registered Nurse who is licensed to work in New Mexico and has Case Management experience.
Job Summary
The Registered Nurse assumes responsibility and accountability for incorporating the vision, values, mission, and critical goals of the organization into their job performance.
Key Responsibilities
- Assume responsibility for Case Management services, ensuring high-quality patient care and outcomes.
- Develop and implement individualized care plans, collaborating with healthcare teams to achieve optimal patient results.
- Conduct thorough assessments, identifying patient needs and developing strategies to address them.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
Requirements
- RN Registered Nurse License/Certification
- 2 years of recent experience as a Registered Nurse RN
- 1 year of recent experience specializing in Case Management
- Current Registered Nurse license to work in the state of New Mexico
- Current Basic Life Support certification
