Job Summary:
Constellation Health Services is seeking a compassionate and empathetic Licensed Clinical Social Worker (LMSW) to join our hospice team. As a field-based Social Worker, you will provide clinical social work services to our patients and their families, promoting their physical, emotional, and spiritual well-being.
Responsibilities:
- Conduct thorough social evaluations to identify patients' and families' needs, strengths, and challenges.
- Develop and implement individualized treatment plans to address patients' and families' needs, including education on disease process, medications, and plan of care.
- Maintain accurate and timely documentation of patient interactions, including social evaluations, treatment plans, and progress notes.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and families.
- Provide bereavement counseling and support to patients and families as needed.
Requirements:
- LMSW license in the state of New Jersey with at least 1 year of clinical experience.
- Hospice and/or bereavement counseling experience required.
- Reliable transportation and willingness to commute within a designated territory.
