The Utah Department of Health and Human Services, Division of Correctional Health Services, is seeking a Care Navigator (Caseworker II) to join our team of skilled professionals.
Job Summary:
We provide evidence-based, individualized interventions to safely stabilize a wide range of mental health issues and help individuals maintain their best level of functioning while in custody.
Key Responsibilities:
- Develop and implement high-quality, evidence-based programs aimed at improving individual outcomes and enhancing community safety.
- Implement strategies to promote accountability and strengthen partnerships while maintaining a positive work environment.
- Collaborate with multidisciplinary teams to provide comprehensive care and support services to offenders.
- Conduct risk assessments and develop individualized treatment plans to address substance use disorders and mental health issues.
- Provide education and training to offenders on topics such as substance use disorder treatment, mental health, and life skills.
- Monitor and evaluate program effectiveness, making data-driven decisions to improve services and outcomes.
Requirements:
- Licensure as a Social Service Worker or Certified Substance Use Disorder Counselor or working towards getting their licensure.
- Experience working with substance use disorder and/or mental health treatment.
- Experience teaching educational classes.
- Experience in case management principles.
- Effective speaking, reading, and written communication skills.
- Knowledge of risk assessments and criminogenic risk factors.
- Familiar with medication treatment for opioid use and other substance use disorders.
Preferred Qualifications:
- Current licensure as a Social Service Worker or Certified Substance Use Disorder Counselor.
