Job Description: Registered Nurse - Home Therapies

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Therapies team. As a Dialysis Nurse Educator, you will be responsible for providing one-on-one training and education to patients with end-stage renal disease (ESRD). You will work closely with patients and their families to develop personalized treatment plans and ensure seamless transitions between clinic and home settings.

Key Responsibilities:

  • Provide patient education and training on dialysis therapy, including proper technique and medication management
  • Collaborate with multidisciplinary teams to develop individualized treatment plans
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed
  • Communicate effectively with patients, families, and healthcare providers to ensure continuity of care
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Required Skills and Qualifications:

To succeed in this role, you will need:

  • A current RN license in good standing
  • At least 1 year of experience in nephrology nursing or a related field
  • Knowledge of dialysis therapy, including peritoneal and hemodialysis
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
