Wraparound Facilitator

Overview:
We are looking for a highly skilled professional to fill the role of a Wraparound Facilitator - Children's Services. As a valued member of our team, you will collaborate with children, families, and support systems to develop comprehensive plans of care and ensure that services are coordinated effectively. Your expertise in facilitation and coordination will enable you to convene Family Team Meetings and guide teams in developing safety plans. A key aspect of this role is providing consultation and support to teams in implementing plans of care and monitoring progress toward goal achievement. With a strong focus on family-centered and strength-based approaches, you will facilitate care delivery in a culturally competent manner. This role requires effective interpersonal skills, attention to detail, and the ability to work varying hours.

The ideal candidate will have a Bachelor's degree in a related field, such as psychology, social work, or counseling, and at least two years of experience coordinating care, facilitating family meetings, and working with diverse populations. Strong communication and problem-solving skills are essential, as well as proficiency in Microsoft Windows and internet applications. We offer a competitive salary range of $58,000 - $68,000 per year, depending on qualifications and experience.
