Job Summary:
As a Home Health RN with CoreMedical Group, you will be responsible for providing comprehensive nursing care to clients in their homes. This includes assessing clients' physical and emotional needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care coordination.
Responsibilities:
- Provide high-quality patient care in a home setting, including assessments, medication management, and wound care.
- Develop and implement individualized care plans that meet clients' unique needs and goals.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
- Communicate effectively with clients, families, and caregivers to ensure understanding and compliance with care plans.
- Document client progress and care provided in accordance with company policies and regulatory requirements.
