Key Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health will:

  • Conduct thorough assessments and evaluations of patient care needs, focusing on functional status, mobility, endurance, pain, and other medical conditions.
  • Develop tailored treatment plans based on patient evaluations, implementing interventions to enhance well-being and monitor progress.
  • Assist physicians in formulating comprehensive therapy plans.
  • Observe and communicate patient responses to treatments and any changes in their conditions to the supervising nurse or physician.
  • Administer treatments aimed at alleviating pain, restoring function, and maintaining optimal physical capabilities, adhering to established standards of practice.
  • Educate and inform healthcare team members, patients, and their families about the care plan, functional limitations, and progress towards therapeutic goals.
Licensure Requirements

To qualify for this position, candidates must possess:

  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification.
  • A valid driver's license and vehicle insurance, along with access to a reliable vehicle.
