Position Overview:
As a Rehabilitation Specialist, you will be instrumental in delivering exceptional care to our patients. Your role will encompass the evaluation, planning, administration, and oversight of Physical Therapy treatments, adhering to physician referrals and established standards.

Key Responsibilities:
1. **Patient Evaluation:** Assess patients' functional capabilities and limitations to determine the necessity for intervention. Develop a comprehensive plan of care with measurable and patient-centered goals.
2. **Treatment Implementation:** Execute individualized treatment plans that consider the patient's physical, social, and cultural needs. Continuously evaluate the effectiveness of treatments and adjust plans as necessary.
3. **Documentation:** Maintain accurate and timely documentation in accordance with professional guidelines, reflecting skilled interventions and patient progress.
4. **Communication:** Effectively communicate patient information and treatment results to the healthcare team, ensuring continuity of care.
5. **Adaptability:** Demonstrate flexibility in managing caseloads and assist in various patient care areas as needed.
6. **Supervision:** Provide appropriate oversight and guidance to support staff, ensuring quality patient care delivery.
7. **Operational Participation:** Engage in departmental meetings and contribute to process improvements, meeting productivity standards.

Qualifications:
Must possess a degree from an accredited Physical Therapy program and hold a valid state license. BLS certification is required. Candidates should exhibit clinical competence and relevant experience.
