The Transitional Care Clinic is committed to delivering high-quality, compassionate transitional care aimed at supporting at-risk patients. Our interdisciplinary team works collaboratively to ensure patients receive the necessary guidance and resources to navigate their healthcare journey effectively.

Patient Care Focus

In this role, you will engage with patients post-acute care crisis, facilitating their transition to long-term primary care. Our goal is to empower patients and help them manage their health effectively over a typical follow-up period of approximately six months.

Work Schedule

This position operates Monday through Friday during daytime hours, with no requirements for nights, weekends, or holidays. On-call duties are shared among team members, averaging about nine weeks per year, ensuring that coverage is manageable and structured.

Key Responsibilities

  • Conduct thorough assessments of patients' health by gathering health histories and performing physical examinations.
  • Deliver direct clinical care and manage a diverse range of health issues.
  • Prescribe medications and therapies, as well as order and interpret diagnostic tests.
  • Evaluate patients to identify their ongoing healthcare needs.
  • Provide comprehensive education to patients and their families regarding health management.
  • Document all patient interactions and care provided in the medical record accurately.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Engage in quality improvement initiatives and contribute to research relevant to patient care.

As an Advanced Practice Provider, you will also play a vital role in mentoring graduate students and contributing to the education of both staff and external groups in your area of expertise. Continuous professional development and maintaining certifications are essential components of this role.
