Job Summary

As a Licensed Practical Nurse at Dartmouth Health, you will play a vital role in ensuring the general well-being of residents through the delivery of high-quality services in a manner consistent with the philosophy of assisted living and in compliance with the New Hampshire Nurse Practice Act and Resident Bill of Rights.

Key Responsibilities

Participate in conducting and maintaining resident assessments using the state assessment tool for assisted living.

Promote accuracy and participate in the process of monthly medication records.

Plan and delegate work assignments during shift.

Assist in conducting admission assessments and orienting new residents to the facility, ensuring satisfactory integration into the facility/community.

Demonstrate an appreciation of the heritage, values, and wisdom of the residents and an understanding of the aging process.

Assist in the investigation of resident and employee incidents, identify safety hazards, and institute corrective action in a timely manner.

Share in the responsibility for providing and maintaining an environment that is conducive to the safety and physical and emotional well-being of residents and personnel.

Monitor and maintain records reflecting residents' physical and mental condition. Report significant changes in a resident's condition to their physician.

Document medication supervised and administered and note resident's response. Follow through on verbal medication change orders received from the physician.

Document resident incidents.

Demonstrate ability to function as a team member in sharing responsibilities for the administration of resident services in cooperation with all members of the team.

Assist in orientation of new staff. Provide ongoing training of staff in order to provide high-quality service consistent with the philosophy of assisted living.

Promote and facilitate interactive problem-solving with staff.

Demonstrate basic knowledge regarding Lifeline and PHB system, initiate problem-solving if needed.

Perform other duties as assigned.

Requirements

Graduate of an accredited school of nursing, current New Hampshire LPN/RN license.

Must possess a high degree of interpersonal relation skills and capability of relating to a variety of people and personalities.

Considerable initiative, judgment, and leadership skills are also required.

Must have the demonstrated ability to direct and manage personnel.

Must possess strong organizational and time-management skills.

Must be willing to learn.

Current BCLS certification required within 30 days of hire.

Physical Requirements

Must be able to lift, carry, stand, bend, squat, crawl, reach, and kneel using good body mechanics for sustained periods of time.

Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

Must be in good general health and demonstrate emotional stability in order to cope with the mental and emotional stress of the position.

Must be able to walk approximately four miles daily.

Must be able to lift/pull up to 35 pounds.
