About the Role:
- Provide clinical services to patients in their homes, as defined by state laws and in accordance with the plan of care.
- Conduct initial and comprehensive nursing evaluations, determine primary focus of care, and develop the plan of care within state-specific guidelines.
- Regularly evaluate and re-evaluate patient nursing needs, ensuring timely and accurate documentation.
- Collaborate with physicians and other healthcare professionals to develop and implement the plan of care.
- Ensure patients meet home health eligibility and medical necessity guidelines.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
