About the Role

Amedisys, Inc. is seeking a skilled Care Transitions Coordinator RN/LPN Liaison to join our team. As a key member of our healthcare team, you will be responsible for educating healthcare professionals, patients, and families about home care services.

Key Responsibilities
  • Educate healthcare professionals, patients, and families about home care services.
  • Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices to generate leads.
  • Coordinate patient admissions with the team, including obtaining all required documentation to complete referral to admission process.
  • Conduct on-site visits to assess patient needs and identify barriers to care.
  • Keep accurate records of patient interactions and referrals.
  • Achieve admissions, product, and payor-mix goals.
  • Meet short and long-term target account goals.
  • Document the minimum expectation of sales calls daily in CRM (including pre and post call notes).
  • Maintain a complete, up-to-date record of targeted referral sources in territory in CRM.
  • Attend community events to promote Amedisys.
  • Respond to customer complaints in a timely manner.
Requirements
  • At least one year of experience in a healthcare-related business development/outreach role.
  • Current RN or LPN license, specific to the state(s) you are assigned to work.
  • Reliable transportation, current driver's license, and liability insurance.
