Responsibilities:
  • Conduct comprehensive assessments of service needs to identify client requirements
  • Coordinate services to support health outcomes, including medical, social, community, legal, and financial services
  • Periodically re-evaluate client needs to ensure ongoing support
  • Establish and verify client eligibility for services
  • Collect and document required state and federal client data
  • Develop and implement individualized plans of care in collaboration with clients
  • Complete referrals for Ryan White services and ensure timely submission of billing forms
  • Work collaboratively with medical case managers and supervisors to ensure seamless service delivery
Requirements:
