Key Responsibilities- Assess the basic nursing care needs of geriatric residents and develop individualized care plans
- Implement nursing interventions and treatments as ordered by physicians, ensuring timely and safe delivery
- Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
- Participate in the care planning process and update care plans as needed
- Document all assessments and interventions accurately and timely
- Current LPN license in the state of employment
- CPR and AED certification, or willingness to obtain within six months
- IV certification, or willingness to obtain within six months
- Long-term care experience preferred
- Excellent communication and interpersonal skills
- Ability to stand and walk for long periods, lift heavy objects, and transfer patients
