Job Title: RN - Hospice

Host Healthcare is seeking a skilled Registered Nurse to join our team as a Hospice RN in Muncie, Indiana.

Job Summary:

We are looking for a compassionate and experienced RN to provide high-quality patient care in a Hospice setting. As a Hospice RN, you will work closely with our interdisciplinary team to deliver individualized care to patients and their families.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
  • Conduct thorough assessments, identify patient needs, and make recommendations for care.
  • Collaborate with the interdisciplinary team to coordinate care and ensure seamless transitions.
  • Document patient information, progress, and outcomes in a timely and accurate manner.
Requirements:
  • Current RN licensure in the state of Indiana.
  • Experience in Hospice care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic Life Support (BLS) certification.
