Job Summary
We are seeking a highly motivated and compassionate Physical Therapist Assistant to join our team of healthcare professionals. As a Physical Therapist Assistant, you will work under the direction of a Physical Therapist to provide client services that have been delegated by the PT.
Responsibilities
- Evaluation and Development of Rehabilitative Plan
Assist in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
Preparation of Clinical and Daily Progress NotesParticipate in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted according to company policy.
Therapeutic Exercises and Self-Help DevicesAs directed, perform routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.
Client and Family EducationTogether with the PT, instruct the client and family and assist in teaching exercise programs to the patient and family.
Medication and Adverse ReactionsIdentify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and report same to the PT.
Observation and Recording of ActivitiesObserve, and record activities and findings in the clinical record and report to the PT and the physician any adverse reaction to treatment.
Consultation with PT and PhysicianConsult with PT regarding the outcome of home visits and report physical findings.
Education of Health Team PersonnelInstruct other health team personnel including, as appropriate, Home Health Aides, as well as family members with respect to certain phases of physical therapy, so that they may work with the client.
Preparation of Daily Progress Notes and Summary ReportsWrite daily progress notes on patients, and submit according to company policy, and prepare periodic summary reports as indicated.
Discharge ActivitiesParticipate with staff, clients, and physicians in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
Scheduling of VisitsConfirm, at least weekly, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits.
Additional DutiesAssume other duties deemed appropriate and necessary by the PT and as designated by Registered Nurse Case Management and Nursing Supervisor.
Qualifications
- Education and Licensure
Graduate of an accredited Physical Therapy Assistant program or equivalent education.
Licensure and CertificationLicensed to practice as a PTA in the State of Florida.
Current BLS CertificationCurrent BLS Certification.
Driver's License and VehicleLicensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
Experience and SkillsMinimum of one year's experience in home healthcare preferred.
Electronic Medical Record ExperienceElectronic medical record experience is preferred but not essential.
Physical and Emotional DemandsMeets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
Self-Directed and Observational SkillsMust display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
Communication and Judgment SkillsMust be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
