Wraparound Facilitator

KVC Health Systems is seeking a skilled Wraparound Facilitator to join our team in the Beckley, Mercer, Wyoming, and McDowell area. As a Wraparound Facilitator, you will play a critical role in building a safe, secure, and stable child welfare system that is family-centered and connected to the community.
Key Responsibilities- Collaborate with families to define and engage formal and natural supports
- Work with community partners to ensure resources are utilized effectively and efficiently
- Provide services to biological family members, including parenting visitation and transportation
- Develop and implement individualized treatment plans in accordance with Wraparound principles
- Conduct regular home visits and family team meetings to ensure timely and effective service delivery
- Attend Multi-Disciplinary Team (MDT) meetings and Individualized Education Program (IEP) meetings as required
- Travel to multiple counties and residential facilities to provide services
- Stay up-to-date on child development, parenting skills, mental health, Wraparound, and trauma
- Ensure all aftercare visits are scheduled, attended, and outcomes are reported to the Wraparound Supervisor
- Bachelor's degree from an accredited college or university in a social service-related field
- Minimum two years of experience in children and family services, with a preference for knowledge and experience in Wraparound and trauma
- Valid driver's license and auto insurance
