Nurse Case Manager

Liberty Medical Care Services is seeking an experienced Registered Nurse Case Manager to provide high-quality care to assigned patients.
The ideal candidate will have a strong clinical background in adult, family, or geriatric settings and experience working in home health or hospice. The RN Case Manager will be responsible for conducting assessments, developing care plans, and collaborating with healthcare teams to ensure seamless transitions of care.
Key Responsibilities- Conduct comprehensive assessments of patients to identify their needs and develop individualized care plans.
- Collaborate with healthcare teams, including physicians, specialists, and other healthcare professionals, to ensure effective care coordination.
- Develop and implement care plans that address the physical, emotional, and social needs of patients.
- Monitor patient progress and make adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions of care.
- Document patient information and care plans in the electronic medical record.
- Current, unrestricted Registered Nurse license in the state of assignment or the ability to obtain and submit proof of license renewal every other year.
- Certification as a Registered Nurse by a national credentialing body.
- Strong clinical background in adult, family, or geriatric settings.
- Experience working in home health or hospice preferred.
- Excellent communication and interpersonal skills.
- Ability to work well under pressure and make decisions regarding nursing problems.
