Galax Health and Rehab Center is seeking a Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

**Key Responsibilities:**

* Observe and chart changes in residents' conditions, taking necessary action to ensure their well-being
* Administer prescribed medications and start intravenous fluids, documenting times and amounts on patients' charts
* Provide basic resident care and treatments, such as taking temperatures, blood pressures, and dressing wounds
* Collect samples from residents and perform routine laboratory tests
* Assist residents with bathing, dressing, and maintaining personal hygiene
* Evaluate nursing intervention outcomes and confer with other healthcare team members as necessary
