As a Rehabilitation Specialist, you will play a crucial role in providing tailored rehabilitation services to clients of all ages. Your responsibilities will include developing individualized care plans that encompass essential education and support for both clients and their caregivers, continuous monitoring of progress, hands-on therapeutic interventions, and effective resource management. You will also oversee the work of support staff and collaborate closely with management to address any relevant concerns.
Work Schedule: This role requires a commitment of 80 hours over a two-week period, with 8-hour shifts during the day and no weekend work.

As a Rehabilitation Specialist, your key responsibilities will include:
- Planning: Continuous development and execution of personalized care plans.
- Organizing/Staffing/Directing: Coordination of rehabilitation services.
- Control: Maintaining accurate and timely clinical documentation for all clients in accordance with agency policies.
- Professional Intervention and Development
- Supervising support staff.
Required Qualifications:
- Current licensure as a Physical Therapist in Minnesota.
- Minimum of one year of experience in acute care, long-term care, or clinical settings.
Preferred Qualifications:
- Doctorate in Physical Therapy.
- One year of experience in home health care.
- Basic Life Support (BLS) certification.
