  • Planning: Continuous development and execution of personalized care plans.
  • Organizing/Staffing/Directing: Coordination of rehabilitation services.
  • Control: Maintaining accurate and timely clinical documentation for all clients in accordance with agency policies.
  • Professional Intervention and Development
  • Supervising support staff.

Required Qualifications:

  • Current licensure as a Physical Therapist in Minnesota.
  • Minimum of one year of experience in acute care, long-term care, or clinical settings.

Preferred Qualifications:

  • Doctorate in Physical Therapy.
  • One year of experience in home health care.
  • Basic Life Support (BLS) certification.
