Job Title: Hospice Dietician PRN

At Primecare Home Care Services Inc, we are seeking a skilled and compassionate Hospice Dietician PRN to join our team. As a Hospice Dietician PRN, you will play a vital role in providing dietary education to our agency staff and serving as a member of the Interdisciplinary Group (IDG).

Key Responsibilities:
  • Educate staff on dietary issues, focusing on end-of-life care and nutritional needs.
  • Assist physicians and nursing personnel in determining patient dietary needs.
  • Develop nutritional plans of care in collaboration with the IDG.
  • Provide dietary assessments and counseling to patients and families as needed.
  • Function as a resource for agency staff and participate in Quality Assurance and Performance Improvement (QAPI) activities.
  • Maintain accurate and detailed clinical records.
Requirements:
  • Licensed as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) in the state of Georgia.
  • At least one year of experience in a healthcare setting.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
