About the Role:

This Renal Care Specialist position offers a unique opportunity to make a meaningful impact in the lives of patients with complex kidney disease. Job Description:

This role involves working closely with patients, nephrology partners, and other healthcare stakeholders to coordinate and facilitate quality, consistent, and cost-effective care. Key responsibilities include:

  • Conducting comprehensive health assessments to identify patients' needs and develop personalized care plans
  • Managing complex patients with chronic kidney disease, focusing on reducing hospitalizations and improving health outcomes
  • Collaborating with healthcare teams to ensure seamless transitions of care and optimal patient outcomes
Requirements:

To succeed in this role, you will need:

  • A minimum of two years' experience as a Nurse Practitioner (NP) or equivalent clinical experience
  • Current unrestricted NP license to practice in the state
  • Current DEA license required or eligible to obtain within 90 days of hire
  • Current CPR or BLS certification
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
