Job Summary

Key Responsibilities:

  • Work collaboratively with physical therapists, occupational therapists, and other healthcare professionals to develop and implement individualized treatment plans.
  • Assist with patient evaluations, assessments, and interventions to promote optimal rehabilitation outcomes.
  • Provide direct patient care, including exercises, modalities, and other interventions as prescribed by physical therapists.
  • Conduct patient education and training on proper body mechanics, exercise techniques, and other relevant topics.
  • Maintain accurate and detailed records of patient progress and treatment plans.

Requirements:

  • Graduate of an accredited Physical Therapy Assistant program.
  • Licensed in the state where the SNF is located.
  • Previous experience in a rehabilitation setting, preferably in a SNF environment.
