Job Summary

We are seeking a skilled Nursing Care Specialist to join our team at Lancaster General Health. As a Nursing Care Specialist, you will provide high-quality patient care and support to our patients in a fast-paced primary care setting.

Key Responsibilities

* Provide clinical advice by phone/electronic correspondence based on clinical judgments and/or triage protocols in accordance with regulatory requirements and standards of practice.
* Consult with providers as necessary.
* Provide patient-specific education including but not limited to preventive care, chronic disease management, acute diagnosis, medications, and other areas pertinent to patient outcomes.
* Prepare patients for visits by using appropriate rooming standards tools.
* Administer injections and medications as prescribed by providers and in accordance with department protocols, regulatory requirements, and standards of practice.
* Prepare and assist providers with various procedures, minor surgeries, and other exams as designated by providers.
* Perform Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
* Collect blood or other laboratory specimens and prepare them for further testing.
* Authorize prescription refills as instructed by providers.
* Assist with test tracking and monitoring overdue results.
* Remove sutures and change dressings on wounds as necessary.
* Ensure exam rooms are adequately supplied and cleaned after each patient visit. Maintain and clean medical instruments and equipment after each procedure.
* Assist providers and patients in reviewing and revising care plans and assess patient progress. Follow up with patients between visits as necessary.
* Assist with proactive outreach for patients requiring preventive and chronic disease services.
* Perform care coordination activities among specialists and transitions of care to obtain all necessary patient information.
* Assist in coordination of referrals, education, and community resources.
* Actively participate in performance improvement initiatives to improve overall patient experience and quality of care.
* Participate in daily huddles with providers.

Requirements

* Completion of a LPN or RN educational program
* Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing
* AHA CPR/BLS certification required within 30 days of hire

Preferred Qualifications

* One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office
* Prior triage, emergency room, cardiac care, intensive care, or other related specialty nursing experience
