Job Summary:

This position is responsible for optimizing patient care by promoting rational, safe, and cost-effective pharmaceutical care. The successful candidate will conduct prospective drug reviews, assess medication prescriptions, and ensure accurate and efficient compounding and dispensing of medications. Additionally, the pharmacist will provide accurate and timely drug information to healthcare professionals, patients, and the public, and collaborate with the healthcare team to manage drug therapy, protocols, and guidelines.

Key Responsibilities:

• Conduct prospective drug reviews on all prescriptions processed
• Independently review and assess medication prescriptions to identify, prevent, and resolve potential and actual drug-related problems
• Ensure safe and appropriate and cost-effective medication therapies for patients
• Provide accurate and timely drug information to healthcare professionals, patients, and the public
• Collaborate with the healthcare team to manage drug therapy, protocols, and guidelines

Requirements:

• Bachelor's degree in Pharmacy or Doctor of Pharmacy degree (Pharm D)
• Licensed or eligible for licensure as a Registered Pharmacist in the state of PA at hire or sit for boards and obtain licensure within 120 days
• Hospital experience preferred
