End-of-Life Care Specialist

At Liberty Hospice, we understand the unique needs of our patients and families facing terminal illness. Our goal is to provide state-of-the-art care and pain management services, delivered by our specially trained staff with emphasis on strength, dignity and compassion.
We are currently seeking an experienced Full-Time Hospice Registered Nurse (RN) to join our team in Chatham & Lee Counties. As a member of our team, you will be responsible for providing high-quality patient care, coordinating physical care with family, patient, physician and Hospice team, and communicating effectively with patients/families and all agency personnel.
Key Responsibilities:
- Apply your knowledge of nursing theories and concepts to deliver comprehensive patient care.
- Document patient care that meets reimbursement guidelines.
- Coordinate patient care with family, patient, physician and Hospice team.
- Communicate effectively with patients/families at all levels.
- Collaborate with all agency personnel to ensure seamless patient care.
Requirements:
- Current RN licensure in the state of practice.
- Successful completion of a nursing education from an approved school of nursing with two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
- Knowledge of home health and hospice care program framework, function, and organization, as well as the nurse's role as a member of the patient care team.
- Working knowledge of nursing principles and practices according to the nursing practice act, agencies' policies and procedures, and reimbursement guidelines.
- 12 hours of continuing education annually.
- CPR certification.
- Ability to accept travel assignments based on agency need, as well as weekend/evening and holiday call assignments.
We offer a competitive salary of $$93,000 per year, which reflects the location and the level of expertise required for this position.
