Speech Language Pathologist

As a Home Health Speech Language Pathologist with CenterWell Home Health, you will play a vital role in providing personalized care to patients in the comfort of their own homes. Our team is dedicated to delivering high-quality, patient-centered care that addresses the unique needs of each individual.

• Evaluate and direct speech/language pathology services to patients in their homes or facilities
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care
• Utilize professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing
• Administer and interpret diagnostic tests and applications of therapeutic treatments
• Observe, record, and report changes in patient condition and response to treatment
• Provide instruction and training to patients in the use of alternative communication systems
• Offer counsel and instruction to patients, families, and healthcare staff
• Maintain and submit documentation as required by the Company and/or facility

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum of six months experience as a speech therapist/speech language pathologist
• Home Health experience a plus
• Current and unrestricted license
• Current CPR certification
• Good organizational and communication skills
• A valid driver's license, auto insurance, and reliable transportation are required
