Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our Home Health and Hospice team in Clovis, NM. As a Home Health and Hospice Nurse, you will be responsible for providing high-quality patient care to individuals in their homes, focusing on their physical, emotional, and spiritual well-being.

Responsibilities:

  • Provide direct patient care to an assigned group of patients during a shift, utilizing the nursing process to assess, plan, implement, and evaluate patient care.
  • Prioritize the delivery of direct nursing care using time and resources efficiently, integrating a standards-based framework model.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the patient's physical, emotional, and spiritual needs.
  • Conduct thorough assessments of patients' conditions, identifying areas of concern and developing strategies to address them.
  • Administer medications, treatments, and other interventions as prescribed by physicians, ensuring patient safety and comfort.
  • Document patient information, progress, and outcomes in a timely and accurate manner, maintaining confidentiality and adhering to HIPAA guidelines.
  • Participate in ongoing education and training to enhance clinical skills and knowledge, staying up-to-date on best practices and industry developments.

Requirements:

  • Current licensure as a Registered Nurse in the State of New Mexico or Compact State.
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) degree.
  • Basic Life Support (BLS) certification required at time of hire.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, prioritizing tasks and managing time efficiently.
  • Flexibility to work varied shifts, including days, evenings, and weekends, as needed.
