We are seeking a highly skilled Social Worker to join our Primary Care team at AdventHealth Shawnee Mission. As a Social Worker, you will play a vital role in enhancing the development, problem-solving, and coping capacities of our patients, promoting effective and humane operation of systems that provide resources and services, and linking patients with systems that provide them with resources, services, and opportunities.
Key Responsibilities- Function as a treatment team member, interpreting social, psychological, emotional, and family system problems and strengths to other members of the treatment team.
- Prioritize clinical problems, formulate treatment goals, and construct treatment plan, revising as needed, based on continuous evaluation and assessment of progress.
- Incorporate patient, physician, customer needs and concerns into decision-making.
- Demonstrate ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability, or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.
- Manage crisis situations as needed, demonstrating ability to quickly appraise crisis situation and select appropriate intervention.
- Master's Degree in Social Work or closely related field, Required.
- One (1) to Three (3) years of position-related experience, Preferred.
- Kansas Licensed Master Social Worker (LMSW) OR Kansas Licensed Specialist Clinical Social Worker (LSCSW), Required.

We offer a full-time schedule with Monday – Friday 8:00am – 5:00pm, hours may vary. You will be required to travel to College Blvd., Lawrence and Ottawa Primary Care Clinics as necessary.
