Part-Time LPN Home Health Nurse in Hutchinson, MN

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services to clients in Hutchinson, MN and surrounding areas. This is a part-time position with flexible scheduling, Monday to Friday between 8 am and 5 pm.
About the RoleAs an LPN at Adara Home Health, you will be responsible for delivering comprehensive client care as assigned by the Case Manager. You will perform functions that require substantial specialized knowledge, judgment, and skill based on the principles of psychological, biological, and social sciences. Your duties will include planning, providing, and evaluating nursing care in accordance with established policies and procedures. You will also recognize and interpret symptoms, institute remedial measures within the legal limits of Licensed Practical Nursing, and report significant changes in the client's condition to the Case Manager immediately.

- Document observations and teachings completely, accurately, legibly, and concisely
- Administer medications and treatments as ordered by the physician and delegated by the Case Manager
- Maintain accurate clinical records and prepare clinical and progress notes
- Assist clients in arranging medical appointments and contacts with appropriate community resources as directed by the Case Manager
- Maintain a safe client environment and identify and report any accidents, injuries, or suspected vulnerable client abuse, neglect, or financial exploitation
To be successful in this role, you must have:
- A current LPN license in the state of Minnesota with no limitations or restrictions
- At least one year of experience in a healthcare setting, preferably acute hospital or home care experience
- The ability to make appropriate nursing judgments and demonstrate strong interpersonal communication and teaching skills
- Strong organizational skills and the ability to work independently
- Excellent written and oral communication skills
