- Wound Care and Management: Provide wound care and management services, including dressing changes and wound assessments.
- Education and Support: Educate patients and their families on proper home health care procedures and strategies to promote independence and self-care.
- Documentation and Communication: Maintain accurate and detailed records of patient care and communicate effectively with healthcare team members to ensure seamless care coordination.
- Education: Associate degree in nursing required; Bachelor's degree in nursing preferred.
- Licensure: Current and valid California Registered Nurse License.
- Experience: Prior nursing experience, specifically in home health care, preferred.
- Certifications: Current CPR certification required.
