Key Responsibilities:- Provide ongoing relationships with patients through access, continuity, education, and comprehensive care
- Diagnose and treat patients at the health center, adhering to PCMH principles
- Support organization priorities of patient care, quality, education, and sustainability for elder adults
- Collaborate with a multi-disciplinary team of physicians and support staff to provide high-quality care
- Provide timely follow-up on patients seen, including chart notations, lab review, and referrals to specialists
