Job Title: Registered Nurse Home Health

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team in providing high-quality home health care to patients in Essex County, NJ.

Job Summary:

The successful candidate will be responsible for assessing and evaluating the needs of patients with multiple disease processes and co-morbidities, developing and implementing individualized care plans, and providing skilled nursing care to patients in their homes.

Responsibilities:
  • Assess and evaluate patient needs, develop and implement individualized care plans, and provide skilled nursing care to patients in their homes.
  • Monitor patient progress, update care plans, and document all changes in patient condition.
  • Identify teaching needs and plan appropriate strategies for individuals and families.
  • Establish and maintain professional working relationships with healthcare providers and other members of the interdisciplinary team.
  • Adhere to agency infection control and safety policies, as well as organizational policies regarding absenteeism and appearance.
Qualifications:
  • RN with Bachelor's degree in Nursing preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills, strong computer skills, and excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
