Job Title: Physical Therapist - Home Health

LHC Group is seeking a highly skilled Physical Therapist to join our team in Red Bluff, California.

Job Summary:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in their homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Evaluate patients' functional status and physical therapy needs.
  • Develop and implement individualized treatment plans to improve patients' mobility, endurance, and overall well-being.
  • Provide hands-on therapy to patients, including exercises, stretches, and other interventions to promote healing and recovery.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current Physical Therapy licensure in the state of California.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
