Healthcare Matron

Position Overview:

As a Nurse Practitioner specializing in Home Risk Assessments, you will play a crucial role in delivering comprehensive clinical services to at-risk health plan members. Your expertise will empower primary care providers and enhance the quality of care for individuals in their own environments.

Key Responsibilities:

• Conduct thorough Adult/Geriatric Assessments, including medical history, diagnosis, treatment, and health education.
• Collaborate effectively with physicians, case managers, social workers, and family members to ensure holistic patient care.
• Perform Home Visits, Skilled Facility Visits, and Telehealth Consultations to meet patients where they are.
• Document all findings and follow-up actions in accordance with Matrix guidelines and protocols.
• Engage in patient education and provide necessary referrals for ongoing care.

Work Environment:
This position offers flexibility with scheduling, allowing you to work in various settings including home visits and telehealth consultations. You will have the opportunity to impact the health and well-being of individuals in your community.

Requirements:

• Strong computer skills and familiarity with electronic health record systems.
• Ability to adapt to frequent changes in a dynamic healthcare environment.
• Valid state driver’s license and proof of adequate automobile insurance coverage.
