Job Title: Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team at LindenGrove Mukwonago, an Illuminus Community. As a Registered Nurse, you will play a vital role in providing holistic, transformative care to our residents.

Job Summary:

The Registered Nurse will be responsible for identifying, providing for, and accepting responsibility for the fulfillment of residents' total nursing needs. This includes assessments, medication administration and monitoring, documentation, and ensuring the provision of person-centered care.

Responsibilities:
  • Maintains and actively promotes effective communication with all individuals.
  • Maintains a positive image of the entity in the community, keeping in alignment with our mission, vision, and values.
  • Coordinates and delegates care of the residents, ensuring compliance with local, State and Federal regulations and administrative policies.
  • Assures quality of care, adhering to best practices around infection prevention and control provided by the nursing department.
  • Responsible for routine checks of residents who are ill or have problems, noting any changes in condition and assisting staff in responding to these changes.
  • Conveys accurate and adequate assessment information to physicians, residents, responsible parties, and staff as medical treatments and evaluates their effectiveness.
  • Responsible for the support, development, and supervision of certified nursing assistant and medication administration assistant personnel.
  • Investigates any incident report or error in procedure through the quality assurance and process improvement methodology.
  • Attains and maintains adequate personnel and supplies in the nursing department.
  • Assesses resident care needs and assists in the development of individual plans of care as needed.
  • Changes staff assignments as needed and attains replacements as needed.
  • Participates in committee meetings and interdepartmental meetings that align with Quality Assurance Process Improvement Standards.
  • Will follow all department safety rules.
  • Performs other duties as assigned.
Requirements:
  • Licensed Registered Nurse in the State of Wisconsin.
  • Active CPR.
  • Knowledge and/or experience in preferred.
  • Knowledge and/or experience with state and federal regulations to quality outcomes and services for all individuals.
  • Ability to relate well to all individuals.
  • Ability to maintain and protect the confidentiality of resident information.
  • Ability to exercise independent judgment and make sound decisions.
  • Ability to adapt to change.
