Job Summary

We are seeking a compassionate and skilled Hospice Chaplain PRN to join our team. As a Hospice Chaplain PRN, you will provide spiritual care and bereavement counseling to patients and their families, ensuring that their needs are addressed in the patient's plan of care.

Responsibilities
  • Perform spiritual bereavement assessments of patients and families/caregivers in a timely manner
  • Provide direct spiritual care and bereavement counseling to patients and families/caregivers
  • Document patient services accurately and timely
  • Attend group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
  • Provide consultation, education, and support to the interdisciplinary group on spiritual care and bereavement counseling
  • Participate in Continuous Care and on call, if necessary
Requirements
  • Graduate of an accredited seminary or school of theology and/or a certificate of ordination or commission from a religious faith group
  • Previous hospice experience working with death and dying strongly preferred
  • Comfortable in an interfaith setting and able to accept different lifestyles, cultures, beliefs, and values
