About the Role

Amedisys, a leading home health and hospice company, is seeking a skilled Hospice Liaison to join their team. As a key member of the sales team, you will be responsible for developing and maintaining relationships with healthcare professionals, patients, and families to promote Amedisys' home care services.

Key Responsibilities
  • Educate healthcare professionals, patients, and families about home care services and their benefits.
  • Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices, to generate leads and drive business growth.
  • Coordinate patient admissions with the team, ensuring seamless transitions and high-quality care.
  • Conduct on-site visits to assess patient needs and identify barriers to care, providing personalized solutions to meet their unique requirements.
  • Keep accurate records of patient interactions and referrals, ensuring data-driven decision-making and continuous improvement.
  • Achieve admissions, product, and payor-mix goals, driving business growth and revenue expansion.
  • Maintain a complete, up-to-date record of targeted referral sources in the territory, ensuring effective outreach and engagement.
  • Attend community events to promote Amedisys and its services, fostering strong relationships with local healthcare professionals and patients.
  • Respond to customer complaints in a timely and professional manner, ensuring exceptional customer service and satisfaction.
Requirements
  • At least one year of experience in a healthcare-related business development or outreach role, with a strong understanding of the home care industry.
  • Reliable transportation, a current driver's license, and liability insurance, with a willingness to travel 50% or more within the assigned territory.
