We are seeking a skilled Licensed Practical Nurse to join our team at Laurelwood Health Care Center. As a Night Shift LPN, you will be responsible for providing high-quality patient care and ensuring the well-being of our residents.

Key Responsibilities:
  • Assess and monitor patients' conditions, preparing and administering medications as needed.
  • Direct the daily activities of Certified Nursing Assistants (CNAs) and ensure adherence to policies and procedures.
  • Participate in admissions, discharges, and transfers of residents, maintaining accurate records and communicating effectively with the healthcare team.
  • Develop and update care plans, prioritizing residents' needs and collaborating with the interdisciplinary team.
  • Report any changes in residents' conditions to the nurse supervisor, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements:
  • Current unrestricted LPN license in the practicing state.
  • Current CPR/BLS certification.
