Job Overview

We are seeking a dedicated Community Living Skills Trainer to join our team. This role is responsible for providing in-home support, case management, instruction, and training to clients in all areas of daily living.

Key Responsibilities:
  • Client Support Functions:
  • Develop written plans tailored to individual client needs
  • Create work/volunteer opportunities to promote skills development and independence
  • Provide benefits resources and counseling using a wellness and recovery model
  • Teach client rights and self-advocacy skills
  • Assist clients in developing natural networks of support
  • Foster community building and self-sufficiency through on-site activities
  • Link clients to community resources and provide ongoing case management and service coordination
  • Administrative Functions:
  • Complete required documentation, including monthly billing reports, SIRs, case notes, and additional case records
Requirements:
  • Bachelor's degree in Special Education, Rehabilitation Counseling, or related field (preferred)
  • Minimum 2 years' experience working with adults with disabilities, preferably in a community setting

Salary and Benefits

The estimated annual salary for this position is $48,240 - $63,360, depending on experience. Benefits include paid time off, health insurance, retirement plan, and more.
