Job Description

We are seeking a skilled Clinical Pharmacist to join our multidisciplinary team at TriHealth. As an Inpatient Clinical Pharmacist, you will play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients.

About the Role
  • You will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes.
  • You will provide critical expertise in drug therapy management, medication reconciliation, and patient education.
Responsibilities
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis.
  • Establish effective collaborative working relationships with pharmacy leadership, physicians, nurses, pharmacists, and technicians.
  • Promote the correct and appropriate use of medications by maximizing clinical effect and minimizing risks.
  • Communicate findings and recommendations to other healthcare professionals responsible for patient care.
  • Ensure compliance with regulatory requirements and organizational policies and procedures.
Requirements
  • Bachelor's Degree in Pharmacy
  • Active pharmacist license in the state of OH
  • Prescription formula and mathematics for calculating dosage and quantity
  • Working knowledge of all Ohio Board of Pharmacy regulations
  • 1-2 years of experience as a Pharmacist, with inpatient or infusion pharmacy experience preferred
