Job Title: Home Health Registered Nurse

Supplemental Health Care is seeking a skilled Home Health Registered Nurse to join our team in Peoria, Illinois. As a Home Health Registered Nurse, you will provide high-quality patient care and support to individuals in their homes, promoting independence and improving health outcomes.

Job Description:
  • Provide comprehensive home health care services to patients, including assessments, care planning, and implementation of care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Develop and maintain strong relationships with patients, families, and caregivers to ensure patient-centered care.
  • Conduct regular home visits to assess patient needs and provide education on disease management and self-care.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Current Illinois RN License
  • Minimum of 1 year of recent home health nursing experience (private duty does not qualify)
  • OASIS/Medicare Certified Visits Experience
  • Electronic Charting experience
  • American Heart Association BLS
  • Valid U.S. Driver's License
